Physical Therapist plan of care:

Diagnosis: that guides patient’s management.

• Plan of Care: Treatment - application of specialist advice and techniques to encourage healing. Collaborate with patients, family members, payers, other professionals, and other individuals to determine a plan of care that is acceptable, realistic, culturally competent, and patient-centered.

Prognosis: Determine patient’s prognoses.

• Determine when patients need further examination or consultation by a physical therapist or referral to another health care professional.
